Instructions for Immunization Pharmacies

Once you have your BCBSM professional PIN , follow these steps to submit your claims electronically:

  1. To enroll in web-DENIS and the Internet Claim Tool download the Secured Access Application — Immunization Agreement (PDF ) and the Use and Protection Agreement (PDF )

    NOTE: If you are a chain pharmacy, you will need to complete only one Secured Access Application and Use and Protection Agreeement and include each chain location that you want to enroll.


  2. Complete a Trading Partner Agreement (TPA) and a Provider Authorization form. Call us at 1-800-542-0945, and select option #3.

Note: Out-of-state providers and facilities do not qualify for Provider Secured Services.
