How to request member claim and eligibility records

If you are requesting records about yourself or on behalf of another contract member, please submit the proper forms to get:

• Personal records about yourself or another person on your contract
• Records about a living member
  (when you are a third party and not a member on the contract)
• Records about a deceased member

How to request legal records If you are an attorney requesting copies of our records where we are not a party to the lawsuit, a valid subpoena is required.

Subrogation

If you file a lawsuit or an insurance claim against another party due to an injury or medical condition, subrogation allows Blue Cross Blue Shield of Michigan to try to recover medical expenses that it paid relative to that injury.
