Get personal records about yourself or another person on your contract


Do you need information about a specific date of service or range of dates, medical procedure or therapy?

You may be able to access this protected health information from our Member Secured Services website.

  • To obtain eligibility information, log in to Member Secured Services, click Who's covered (Coverage) or What's covered (Benefits) under the Manage my plan tab.
  • To view claim summary or Explanation of Benefit Payments statement information, click Explanation of benefits statement (EOB).

Or, you may call a Customer Service representative to help you with your request. Call the Customer Service phone number on the back of your ID card. If you don't have your ID card, call the Blue Cross Blue Shield of Michigan corporate operator at 313-225-9000. You'll need to provide your enrollee ID number.


Do you need all the records?

If you need your HIPAA designated record set, complete the Request for Access to Designated Protected Health Information Records (PDF). You'll receive a response within 30 days of the receipt of the request.
